When you submit personal information to Health Forum Online, you understand and agree that Health Forum Online may access, store, and use your customer profile. Health Forum Online will not store credit card information nor will it disclose credit card information to any third parties.

Health Forum Online, LLC will use your information only for that specific purpose for which you submitted the information. We DO NOT sell or rent any personally identifiable information about you, including but not limited to, your emails, addresses, names, and phone numbers, to any third party without your permission. Unless otherwise indicated, the information provided by you to Health Forum Online is for internal use only to fulfill your order requirements or for administrative purposes. We may disclose your personal information we collect, as described above, to companies that perform services on our behalf to fulfill order requirements or administrative requirements.

We will cooperate with legal authorities in order to assist in an investigation and may release your personal information to them, as required by law.

Refund, Cancellation and Grievance Policy

HealthForum is fully committed to conducting all activities in strict conformance with the ethical principles and guidelines of all of our accrediting bodies. As such, HealthForum will comply with all legal and ethical responsibilities to be non-discriminatory in promotional activities, program contents and in the treatment of program participants. The monitoring and assessment of compliance with these standards will be the responsibility of the Director of HealthForum's Continuing Education Program in consultation with the members of its Planning Committee.

A full refund will be given to prepaid registrants/participants in the event that a HealthForum CE activity is canceled.

While HealthForum goes to great lengths to assure fair treatment for all participants and attempts to anticipate problems, there will be occasional issues that come to the attention of the activity staff that requires intervention and/or action on the part of the activity staff or an officer of HealthForum. This procedural description serves as a guideline for handling such grievances.

To submit a grievance, please send your name, current contact information, course title(s), order # if applicable, and a brief description of the matter to [email protected].

When an online course participant, files a grievance and expects action on the complaint, the following actions will be taken.

  • If the grievance concerns a course author, the online course content material presented, or the style of course presentation, the participant will submit a grievance via [email protected] and it will be directed to the CE Program Director.
  • The CE Program Director will pass these grievances on to the course author/Planning Committee, assuring the confidentially of the grieved individual.
  • Additionally, the CE Program Director will mediate and serve as the final arbitrator regarding subsequent actions, which may include:
    • attempting to move the participant to another online course, or
    • issuing a credit for a future CE activity, or
    • providing a partial (if processing fee has occurred) or full refund of the course fee

The latter two will require a written note, documentation of the grievance, for record-keeping purposes. The note need not be signed by the grieved individual.

  • If warranted, the CE Program Director  will address broader program-wide concerns/grievances (e.g., user friendliness/interface) with the Planning Committee and modifications will be taken into consideration.
